This form has been modified to change the following:
The description was changed to "Apprenticeship or Training
Program Certification"
At the first check box "material proposal" is replaced by
the "deliver and install proposal".
At the second check box "deliver and install" is inserted
before "following".
A paragraph which starts with "In accordance with the
provisions..." was revised and separated into main paragraph
and sections 1,II, III, and IV for clarity.
A paragraph which starts with "The requirements of this
certification and disclosure..." was revised by adding "The
bidder is responsible for making a complete report and shall
make certain that each type of work or craft job category
that will be utilized on the project is accounted for and
listed. The Department at any time before or after award
may require the production of a copy of each applicable
Certificate of Registration issued by the United States
Department of Labor evidencing such participation by the
contractor any any or all of its subcontractors."
