In order to comply with Federal Regulations, all bridges that may be adversely affected by scour must have a Plan of Action prepared to provide information for use in addressing the safety of the bridges, especially during a significant flood event.  The form provides a standard outline for use by personnel responsible for developing a Plan of Action for structures that have been identified as being scour critical bridges, based on federal guidelines.

This form is completed by bridge inspectors (state, local and consultants) to report that a special feature inspection has been completed or that one is required.

This form is for the contractor to get permission from the property owner to let IDOT's field personnel and geotechnical staff obtain soil samples.
