This form has been modified to:
Replace "greater than 14 feet 6 inches wide and up to and
including 16 feet wide" with "requiring a police and/or
civilian escort" in dot point #9 paragraph one under
Provision C.
Add "and/or 17 feet wide" in dot point #9 paragraph two
under Provision C.
Change form to read from left to right rather than as a
booklet.
