This form has been modified to reflect the following:
Renumbered from LM 6 to BMPR LM6.
Autoprint date added.
Field sizes increased for Producer Name and Location,
Supplier Name and Location, Material Name and Inspector
Name.
A field added for the District Number and field numbers
adjusted accordingly.
PRE has been added to the drop down box selections for "Type
of Inspection".
A drop down box has been added to the "Resp Loc" field.
