This form will serve as basic data collection to serve as a cover document for the submittal of a Highway Safety Improvement Program (HSIP) or High Risk Rural Roads Candidate project to the Central Safety Committee.  This form is required by IDOT Policy SAFETY 1-06 "Highway Safety Improvement Program (HSIP).

Provide greater detail of Erosion & Sediment Control practices inspected and outfalls.

 

Provide area to detail items needing maintenance or corrective action and time limit to make correction before deficientcy deduction will be imposed.

 

Added areas to indicate Area of Disturbed Soil and Areas Ready for Final Cover and 70% Coverage Established.
