BLR 05311, Local Agency Amendment for Federal Participation (NEW) is a new form effective January 2006. This form creates a standardized federal-aid amendment for utilization by statewide local agencies when revising funding amounts or allocation of funds.

BLR 05610, Preliminary Engineering Services Agreement for Federal Participation has been updated to include sub-consultants reporting requirements (item 12 under “Engineer Agrees”) including form BLR 05613 as Exhibit B. These changes are effective January 2006.

BLR 04310, Ordinance for Municipal Census (1/06). The ISO reference number has been removed.

BLR 04311, Affidavit for Municipal Census (1/06). The ISO reference number has been removed.

BLR 04312, Affidavit for Municipal Annexation (1/06). The ISO reference number has been removed.
